I agree & authorize Roadrunner Shuttle and Limousine service to use all statements acquired from the Customer Service Questionnaire and images acquired from participation in recieving to be used or reproduced by Roadrunner for print, website testimonial section, limo blogs, future promotional material, and email communications, and for as long as Roadrunner is in business. I hereby release Roadrunner from any liability whatsoever from results of statements received and images acquired from client to and for Roadrunner use. You (listed above) reserve the right to cancel, in writing, your testimonial agreement at any time if you feel you are not represented in a positive image by Roadrunner.

By submitting this disclosure, I agree to participate as a testimonial for Roadrunner, at will, and I have had the chance to ask any questions about disclosure agreement I do not understand.
